At Monarch Ideal Care, we offer bioidentical hormone replacement therapy using only two delivery methods:
Sublingual troches
Subcutaneous pellets
We keep options streamlined so patients receive consistent guidance, predictable follow up, and monitoring that supports safe care.
The "best" option depends on the whole picture:
your symptoms and goals
your medical history and risk factors
your lab results
your comfort with a daily options (troches) vs a longer-duration option (pellets)
Many patients start with troches first, then transition to pellets once the dosing target is clearer and they've experienced how replacement feels in their body.
Sublingual troches dissolve under the tongue and provide bioidentical hormone replacement through a dosing schedule designed for you.
Why I often recommend starting with troches (especially if you're new to hormone therapy):
They have a shorter half-life, which allows for dosing adjustments more easily early on
They're helpful for learning how your body responds before committing to longer-duration delivery methods
Troches are available for both women and men, depending on labs, symptoms and goals.
Typical investment: Troches vary by dose and typically start at $125 for a 30 day supply
Pellet therapy involves placing small, bioidentical pellets under the skin. Pellets release hormones steadily over time.
Dr Leslee is a certified Biote provider, and pellet dosing is determined based on your symptoms, health history, and lab results.
Women's Pellets:
Duration: typically 3-4 months
Options: testosterone only OR testosterone + estradiol
Typical investment: $425
Men's Pellets:
Duration: typically 4-5 months
Options: testosterone only
Typical investment: $725
Pellets can be a great fir for patients who prefer a consistent, long-duration approach and want fewer day-to-day dosing decisions.
Hormone therapy is replacement therapy, not stimulation therapy. We are not forcing your body to make more hormones.
If you discontinue therapy, hormone levels return toward your pre-treatment baseline over time. You're not "locked in", and we can adjust or stop if the plan isn't a fit.
Also—if labs are out of range, correcting them doesn't guarantee symptom resolution. Some symptoms improve quickly, others take time, and some require a different approach altogether. We listen to your body and adjust the plan as needed.
These treatments (troches and pellets) are designed for physiologic and age-related hormone decline in adults. We do not use troches or pellets for transgender therapy at this time.
If you're seeking gender-affirming hormone care, we support your journey and invite you to explore our Primary Care services, where that care is handled through a different clinical framework.
